Contraceptive failure rates in France: results from a population-based survey.

چکیده
BACKGROUND Despite the widespread use of highly effective contraceptive methods in France, one in every three pregnancies is unintended, of which 65% occur while using contraceptives. In the USA, 49% of pregnancies are unintended, half of which result from contraceptive failure. This study provides estimates of method-specific failure rates among French women. METHODS We use data from the 2000 Cocon Study, a population-based cohort, comprising a representative sample of 1689 women aged 18-44 years who described their contraceptive history. Piecewise-constant hazards models were used to estimate method-specific contraceptive failure rates during the first 5 years of contraceptive use. A random effect was introduced to take into account the fact that some women contribute more than 1 contraceptive episode. These same models were used to assess the effects of socio-demographic characteristics on the probability of contraceptive failure among pill, intrauterine device (IUD) and condom users. RESULTS Overall, 2.9% of women experienced a contraceptive failure in the first year of use and 8.4% in the first 5 years of use. The IUD had the lowest first year failure rate (1.1%), followed by the pill (2.4%), the condom (3.6%), fertility awareness methods (periodic abstinence or safe period by temperature) (7.7%), withdrawal (10.1%) and spermicides (21.7%). These failure rates varied little by user characteristics. CONCLUSIONS Lower failure rates among French women compared with those reported for US women suggest differences in contraceptive practices which need to be further explored.

Ectopic pregnancy is again on the increase. Recent trends in the incidence of ectopic pregnancies in France (1992-2002).
BACKGROUND We aimed to assess the recent incidence trends of ectopic pregnancies (EP) in France (1992-2002). METHODS A population-based register of all women aged 15-44 years with EP was set up in Auvergne (central France). We calculated rates of EP as a 'reproductive failure' or as a 'contraceptive failure', frequencies of exposure to the two main risk factors for EP (cigarette smoking and c...
متن کامل[Increase of repeat abortion in France: from contraceptive issues to postponement of childbearing age].
BACKGROUND The number of women resorting to abortion several times has increase continuously in France since 1975, similar to the situation in many countries where contraception is widely available and used. The analysis of time trends in so-called "repeat" abortion across social, demographic and contraceptive characteristics can be helpful in capturing the social meaning of this practice and i...

BACKGROUND Despite the widespread use of highly effective contraceptive methods in France, one in every three pregnancies is unintended. Among women experiencing an unintended pregnancy leading to an abortion, half had changed their contraceptive method in the 6 months preceding the abortion, in most cases switching to a less-effective method or to no method at all. This study provides estimate...
متن کاملContraceptive Failure in the United States: Estimates from the 2006–2010 National Survey of Family Growth
CONTEXT Contraceptive failure rates measure a woman's probability of becoming pregnant while using a contraceptive. Information about these rates enables couples to make informed contraceptive choices. Failure rates were last estimated for 2002, and social and economic changes that have occurred since then necessitate a reestimation. METHODS To estimate failure rates for the most commonly use...
متن کاملTypical-use contraceptive failure rates in 43 countries with Demographic and Health Survey data: summary of a detailed report
BACKGROUND While most unintended pregnancies occur because couples do not use contraception, contraceptive failure is also an important underlying cause. However, few recent studies outside of the United States have estimated contraceptive failure rates, and most such studies have been restricted to married women, to a limited number of countries and to 12-month failure rate estimates. METHOD...
